The Opportunity

 

The Care Coordinator will be part of our amazing Amplar Health Team. In this role you will be coordinating clinical support to participants who may have recent hospital admission and/or complex chronic illnesses. This includes supporting a caseload of participants (managed virtually, i.e. work from home).

 

Drawing on your holistic knowledge and background in health, you will assess and mitigate patient risks by understanding their key motivators for change. You will utilise a coordinated and integrated approach, facilitating health behaviour change and care coordination in the delivery of health coaching, injury and illness recovery, and chronic and complex disease management programs.

 

This is a full-time, permanent position on a 7-day rotating roster, with shifts scheduled between 7:00 AM and 8:00 PM Adelaide time. Please note that this is a clinical role with no direct patient contact.  

 

About You 

 

You are an advocate for preventative health and have the natural ability to build rapport with a wide range of clients and healthcare professionals. You have previous experience working in the community environment across a diverse demographic and a solid understanding of Chronic Disease management is essential for this role. Previous experience in care coordination or case management will be highly regarded.

 

Working in this highly autonomous position you will have an organised and flexible mindset that allows you to meet the unique needs of your clients.

To be considered for this role you will be a Registered Nurse with relevant registration with AHPRA.
